F1 Represents restricted stock units ("RSUs").
F2 The RSUs were acquired under the 2012 Equity Incentive Plan. 25% of the RSUs vest on the first year anniversary of the grant with the remainder vesting 6.25% quarterly thereafter.
F3 On March 4, 2021, the Reporting Person filed a Form 4 which inadvertently reported that, 33,000 RSUs were directly awarded under the Issuer's 2012 Equity Incentive Plan. In fact, as reported in this Form 4 amendment, the Reporting Person was directly awarded 29,000 RSUs under the Issuer's 2012 Equity Incentive Plan.
